Laboratories can use the following types of forms to request details regarding the case requirements:
A paper form to be downloaded and completed manually (this can be any file format)
A web form to be completed online
When you create a form, it appears to the Dentist in the Case submission window.
Note:
If you have provided more than one paper form, all paper forms are made available.
Even if you have created more than one type of web form, only one form can be presented to a Dentist at a time.
To access your forms, follow these steps:
In the CS Connect toolbar, click .
In the Settings window, click Forms.
The list of forms is separated into a list of paper forms and a list of web forms.
