Adding Paper Forms for Dental Case Submissions

To add a paper form for Dental case submissions, follow these steps:

  1. Create a form. It can be a document or image.
  2. In a browser, open CS Connect with your Laboratory connection.

  3. In the CS Connect toolbar, click .

  4. In the Settings window, click Forms.

  5. In the File Forms section, click Attach a form.

  6. Browse on your computer to find the form you created, select it, and click Open.
    The selected form appears in your list of paper forms.
